What to Expect During Your HRT Lab Test

 

Our in-office laboratory supports a comprehensive model by providing thorough hormonal, metabolic, renal, hepatic, and immune system evaluation.

 

Blood and urine samples are collected on site, with many chemistries, urinalyses, toxicology studies, and screening tests performed in-office for efficiency and accuracy. When specialized testing is required, specimens are couriered to trusted reference laboratories.   

 

Laboratory evaluation typically includes assessment of the adrenal glands, thyroid, parathyroid, pancreas, gonads, and pituitary, allowing us to evaluate both upstream regulatory hormones and downstream target hormones. Renal and hepatic function are assessed concurrently to ensure that hormone therapy and supportive treatments can be prescribed and monitored safely.

Initial Consultation & Evaluation

The initial HRT consultation includes a detailed medical history, discussion of symptoms and goals, and determination of appropriate laboratory testing. Blood and urine samples are collected on site, with many chemistries, urinalyses, toxicology studies, and screening tests performed in-office.​ If an outside laboratory is preferred, a written laboratory order will be provided.

Medication Arrives

The HRT medication will be mailed directly to you by a licensed compounding pharmacy. Costs for compounded medications are paid directly to the pharmacy and vary based on formulation and dosage.

HRT Treatment Starts

Therapy is typically initiated two to four weeks after the initial consultation, once results have been thoroughly reviewed and interpreted. Treatment plans are individualized and based on objective data rather than symptoms alone. Hormone therapy is never static; it is adjusted based on lab findings and patient feedback.

Ongoing Monitoring & Follow-Up

Ongoing monitoring is a central component of safe and effective hormone therapy. Blood work is commonly repeated at 90-day intervals. This schedule allows us to assess therapeutic response, monitor safety, and identify trends over time. Nutritional supplementation and selected supportive or “natural” therapies may be recommended or dispensed based on laboratory findings and patient response.

What does sex hormone testing evaluate?

Sex hormone testing measures hormones that influence libido, but more importantly, they are essential for the proper evaluation of thyroid and adrenal function. Are hormone tests done fasting?

Some hormone panels require fasting. Dr. Klein will give patients specific instructions beforehand, depending on which tests are ordered.

Why would a man need hormone testing?

Hormone testing evaluates fatigue, low libido, muscle loss, mood changes, metabolic concerns, sleep disturbance, and cognitive changes. These hormones are also critical to understanding thyroid and adrenal disorders and their downstream effects on immune function and bone health.

How do I get my HRT medication?

Hormone replacement medications are prescribed individually and prepared by a licensed compounding pharmacy. These medications are mailed directly to you.

Why would a woman need hormone testing?

Hormone testing assesses menstrual irregularities, menopausal symptoms, mood changes, bone health, sleep disruption, and unexplained weight gain. Men and women require evaluation of the same hormones for the same physiologic reasons, although normal ranges differ substantially between genders.

Does Stages of Life use testosterone injections?

Generally, no. Testosterone injections produce wide hormonal fluctuations and can suppress normal testicular function. This can lead to testicular atrophy and may increase conversion to estradiol, contributing to breast enlargement.

Does Stages of Life use hormone pellets for HRT?

We do not utilize hormone pellets. Pellet therapy is difficult to adjust once implanted, offers limited control over-dosing, and may expose patients to prolonged periods of excessive hormone levels. Pellets are also typically more expensive.

How does transdermal hormone therapy work?

Transdermal hormone therapy delivers hormones through the skin (via patches, gels, or creams), allowing them to enter the bloodstream directly and maintain more stable hormone levels. This route bypasses the liver’s first-pass metabolism, which can reduce systemic side effects compared with oral therapy.

Is hormone testing done with blood work?

Yes. Blood testing is the most precise and clinically meaningful method for hormone assessment. Salivary testing measures hormone levels only in saliva and is not considered reliable for clinical decision-making.

Do I have to use Stages of Life's onsite lab for HRT testing?

No, if an outside laboratory is preferred, a written laboratory order will be provided.
